APRS is focused on providing exceptional physical therapy services at a reasonable cost to the Gallatin Valley and beyond.

Your First Visit

Please arrive 15 minutes before your appointment to fill out intake paperwork, or complete paperwork beforehand and bring it with you.

During your initial visit you will be seen by a Licensed Physical Therapist who will review your history, and all pertinent documentation. The physical therapist will perform a comprehensive physical evaluation to determine the best plan of care.

Your First Visit

What do I need to bring with me?

  • a list of all prescription and over the counter medications, vitamins and supplements you are currently taking
  • your insurance card if applicable
  • a photo ID
  • if this is a worker's compensation claim please bring your claim number and your case manager's contact information
  • if this is an auto insurance claim please bring your claim number and claim adjusters contact information
  • your physical therapy referral if one was given to you (please note that a physician referral is not required to see a physical therapist, however, some insurers do require one)
  • any related documents pertaining to what brings you in for physical therapy: surgical reports, MRI reports, x-rays, etc

How should I dress?
Please wear comfortable, loose-fitting clothing so we can expose the area that we will be evaluating and treating. If you are being seen for orthotics, please wear/bring appropriate shoes to be fit for custom orthotics.

How long will my first physical therapy visit last?
Your first physical therapy visit will take approximately 1 hour.

Payment & Insurance Info

As a courtesy, APRS will bill all General Insurances, Workers' Compensation, Medicare or Motor Vehicle Accident. We strongly encourage our patients to know their benefits and we are here to answer any questions. We are in network with most major insurances. If you do not see your insurance listed below, please give us a call as we may be a preferred provider. For patients without insurance, self-pay options are available; please contact us for additional information.

No Surprises Act

APRS Physical Therapy is in compliance with the No Surprises Act and will provide all cash pay clients with a Good Faith Estimate for Physical Therapy Services. This good faith estimate of expected charges will:

  • be available in a written document that is clear, understandable; and
  • will be orally provided when the service is scheduled or when the patient asks about costs; and
  • available in accessible formats, and in the language(s) spoken by the patient.
